I am seeking advice on a recent crash involving my custom VTOL aircraft. I suspect an issue with the transition settings, but I cannot pinpoint the exact cause of the failure.

Aircraft Configuration
Airframe: Tandem Wing + Hexacopter (The center 2 motors of the hexacopter are configured to tilt for transition).
Flight Controller (FC): Pixhawk Orange Cube. -
Flight Modes Tested Successfully
The aircraft flew stably and without issues in the following manual/semi-automatic modes:
Q_Stbil (Quad Stabilize)
Q_Hover (Quad Hover)
FBW-A (Fixed-Wing Fly-By-Wire A) -
Crash Scenario
The crash occurred in Auto mode
The aircraft took off and climbed to an altitude of 100 meters.
During the transition from hover to forward flight, the aircraft suddenly rolled violently to the right** and crashed to the ground.
The system was unable to maintain roll control during the transition phase.
I have attached the necessary log files and video of the crash for detailed analysis.
