One of the quad motors disabled during transition from FBWA to QHOVER mode

Ground speed generated from POS abnormal during FBWA mode, so decided to switch to QHOVER to land, but the plane flipped, stalled and crashed. Could anyone help me on this? I couldnt find any mine or pixhawk’s mistake there. The log file is here please let me know if anyone found the problem thanks!
