The filter review tool is just being tricked on the “failed state” section of the flight. The batch collection bin size is too small, so you can ignore the filter data for that part of the flight.
With that out of the way, excessive vibes, pushing your craft to motor saturation, and I bet hitting a current limit near 40A that tripped a component (or you were cooking motors, or caused a desync or all of the above at the same time)
Many thanks for your help to analyse it out, this quad was flying wonderfully before a previous crash I think this may affect the FC as well even nothing seen visually but the behaviour become erratic.
